Maternal/Child Health Programs The purpose of the Maternal and Child Health Program is to provide public health nursing services to improve the health status of pregnant women, parents, infants and children in Ozaukee County. These services identify those who have health risks in order to provide early intervention, to prevent occurrence of health problems or to reduce the effects of disability or illness. Post Partum Follow-up The goal of the Post Partum Follow-up service is to facilitate a safe and smooth transition from hospital to home. Our staff addresses the nursing care needs of the new mother and her infant. Through this service, a telephone call is made and a home visit is offered to assess both mother and newborn. This visit includes teaching, counseling, and providing reassurance and anticipatory guidance on various parenting issues. A folder of information on parenting and community resources is provided. If concerns are identified during the initial visit, a follow up visit or referral to an appropriate community resource is offered. Families are referred to Post Partum Follow-up from area hospitals, doctors, and community agencies or by request of the families themselves. The Public Health Department contacts all Ozaukee County families with newborns. Families not referred to Post Partum follow-up receive a packet of community service information. This packet includes information on childhood immunizations and clinic sites, WIC, Healthy Start and a summary of the Public Health Departments services. Prenatal Care Coordination The goal of the Public Health Department is to maintain a healthy pregnancy and improve birth outcomes.Prenatal Care Coordination is a Medical Assistance benefit that helps pregnant women get the support and services they need to have a healthy baby. Pregnant women who are eligible for this benefit are assigned a public health nurse to assess their pregnancy related needs and coordinate medical, social and / or educational follow-up. Presumptive Eligibility (PE) Medical Services for the pregnant woman What is Presumptive Eligibility? Presumptive Eligibility is a one-time temporary Medicaid certification that can allow an uninsured pregnant woman to receive immediate pregnancy-related OUTPATIENT SERVICES while her application for Wisconsin Medicaid is being processed. Who is eligible for Presumptive Eligibility? Pregnant women with a medically verified pregnancy are eligible. Pregnancy can be verified by your doctor or by the Ozaukee County Public Health Department. Her familys gross income cannot exceed 185% of the federal poverty level (income is reported verbally and does not need to be verified). How long does Presumptive Eligibility last? The pregnant womans eligibility is valid from the date she is determined eligible through the end of the month following the month eligibility was determined. Once a woman is found eligible, she MUST apply for Medicaid at Workforce Development Center for these benefits to continue. What services are covered under Presumptive Eligibility? AMBULATORY PRENATAL CARE SERVICES: Doctor visits, immunizations, prescription drugs, lab and X-ray services, Prenatal Care Coordination, and dental services. IT DOES NOT COVER INPATIENT HOSPITAL COSTS, INCLUDING LABOR AND DELIVERY. |
