A service charge of $7.00 per dose is charged for
all children’s vaccines. The service charge can be adjusted based on ability to
pay for any children’s vaccine. We do encourage clients to go to their doctor
for immunizations if they are covered under their health insurance plan.
Children under 18 years old must be accompanied by parent or legal guardian.
Immunization records of children are required. Grandparents cannot sign
consent forms – no exceptions will be made.
