How to Obtain a Passport
The Ozaukee County Clerk is a designated acceptance facility for
obtaining a U.S. Passport. You will not receive your passport from us.
Our role is to accept completed passport applications and fees for
processing.

FEES
PASSPORT BOOK
If 16 years of age or older, the Passport Book processing fee is $75
payable to the “U.S. Department of State”, and the separate execution
fee is $25 payable to “Ozaukee County”.
If 15 or younger, the Passport Book processing fee is $60 payable to the
“U.S. Department of State", and the separate execution fee is $25
payable to “Ozaukee County”.
PASSPORT CARD
The Passport Card is a U.S. Passport issued in card format. Like the
traditional Passport Book, it reflects the bearer’s origin, identity,
and nationality and is subject to existing passport laws and
regulations. Unlike the Passport Book, the Passport Card is valid only
for International travel by land or sea between the United States,
Canada, Mexico, the Caribbean and Bermuda. It is not globally
interoperable and is not valid for travel by air to or from any foreign
destination.
If 16 years of age or older, the Passport Card processing fee is $20
payable to the “U.S. Department of State” and the separate execution fee
is $25 payable to “Ozaukee County”.
If 15 or younger, the Passport Card processing fee is $10 payable to the
“U.S. Department of State”, and the separate execution fee is $25
payable to “Ozaukee County”.
If applicant is under the age of 16, both parents or legal guardians
must show valid identification, sign and take an oath before a passport
acceptance agent.
Separate checks to the U.S. Department of State
and Ozaukee County are required. Our office cannot accept credit cards
or debit cards. By law, the passport processing fee and
execution fees are non-refundable.
REQUIREMENTS
All applicants must appear in person. Along with the passport
application, you must submit the following:
* Proof of U.S. Citizenship
* Proof of Identity
* Photocopy of the Identification Document Presented
* Two photographs
Proof of U.S. Citizenship
A STATE CERTIFIED copy of a birth certificate issued by the County or
State serves as evidence of citizenship. It must have the seal of the
issuing office and the filing or registration date, which must be within
one year of birth.
If you are 18 or older and have an expired passport, you are required to
submit the passport rather than a birth certificate.
When submitting an expired passport or a Certificate of
Naturalization/Citizenship for an applicant under the age of 18, you
must include a STATE CERTIFIED copy of the child’s birth certificate,
which includes the names of the parents. Please see the application for
additional information for obtaining a passport for a child under the
age of 18.
Naturalized citizens must present their original Naturalization
Certificate.
All evidence of citizenship will be returned with your passport.
NOTE: Hospital, baptismal, birth registrations or photocopies of birth
certificates are not acceptable.
Proof of Identity
Identification that includes your signature and physical description or
photograph such as a valid driver’s license or a Federal, State, or
Municipal identification card qualifies as proof of identity.
Photocopy of Identification Document Presented
The photocopy must be of the ID document that you will
present when you apply.The photocopy must be on plain white,
8 1/2 x 11" standard paper stock, showing the front and
the back of your ID. Photocopy must contain images on only
one-side of each page submitted.
If copies cannot be made on the same side of one page, you
may present two separate
pages; one displaying the front of the ID and the second
displaying the back of the ID.
The County Clerk's office will make a copy of your ID document for an additional fee.
Two Photographs
Two professional passport photos (2 x 2 inches) are required. The
Ozaukee County Clerk’s Office does offer this service for a fee of $10.
Digitized photos will be accepted at the discretion of passport
services. School photos are not acceptable.
Complete all requested information on the application form. If all
requirements are not met, the County Clerk’s office cannot accept the
application for processing.
EXPEDITED SERVICE
For normal processing, passports should be received in approximately 4
- 6 weeks.
For faster processing, you may request expedited service and pay an
additional fee of $60. You should receive your passport in approximately
2 - 3 weeks.
ADDITIONAL INFORMATION
Do not sign the passport application until you are instructed to do so
by the Passport Agent in the County Clerk’s office.
Photocopies of the passport application will not be accepted.
