An opportunity to meet individually with an Ozaukee County Bar Association Attorney to create your personalized Power of Attorney for Health Care. Why complete Power of Attorney (POA) documents? POA's allow YOU to choose the individual you would want to make decisions for you in the event that you are unable to make them yourself. If incapacity occurs and you are without one, it will likely be necessary for your family or other to ask the Court to appoint a guardian - a process that can be costly, time-consuming and cumbersome. Call Human Services to schedule you 1/2 hour appointment at 262-284-8200 or 262-238-8200.
