A one-stop center where staff can answer your questions, give information, and provide referrals.  This service is available for Seniors and physically disabled adults (age 18+), concerned family members, and the community.

An information center where you can select from over 150 brochures and pamphlets on programs and services for older adults.

Assistance with  your long-term care needs including options for in-home care, community-based services, alternative housing, and funding programs.  Staff will assist you in understanding all options so you can make an informed decision that fits your situation.

Visits in your home by staff to discuss your need for help; follow-up assistance to find in-home or community services that will meet your needs.

No information about or obtained from an individual participant shall be disclosed in any form identifiable with the individual to any person outside the agency or program involved without the informed consent of the participant or his/her legal representative except:
a.) By court order or
b.) When securing client requested services, benefits, or rights.
