You may apply for new or additional BadgerCare Plus, Medicaid, FoodShare and/or Child Care benefits by visiting ACCESS online.
You may also call the Moraine Lakes Consortium Call Center to request a paper application and schedule any necessary appointments. FoodShare and Child Care requests require an interview to be completed before eligibility for benefits can be determined.
The call center also accepts fraud referrals. There may be situations in which an individual or family is receiving benefits based on fraudulent, false information. When these circumstances are discovered, an investigation is completed. Upon completion of the investigation, there may be a need for the benefit to be repaid and further legal action may be taken in some situations. You may call the call center to report suspected fraud.
